As most of you remember that PMMA injection has been tried in Glans enhancement ; some doctors e.g. in Berlin mentioned that it has a temporary result (which I think the injection was done too deep (deeper than 3 mm of the Glans epithelium) i.e. into the CS into the blood stream, and others like Dr.C. has mentioned to us that there was an unpleasant esthetic result (uneven or flowered-like Glans) ---> And now I have just found the reason and the answer for that unpleasant result and definitely the one who solved this problem were these amazing korean doctors who were the 1st one in the world who did Glans filler injections.

As we know most of the doctors who are doing this technique were injecting the filler into the corona of the Glans :-

en.wikipedia.org/wiki/Corona_of_glans_penis

www.urologicalcare.com/educational-resou...photo/list.php?ID=48

This technique was wrong and there is a new study which proved that (just recently 5 days ago):-

And this is why we get uneven or flowered-like effect of the Glans because of the thickness of the lamina propria layer at this area, these korean doctors have solved this problem by injecting a filler which has a high collagen-stimulating function ( PMMA or Dextranomer also there is a product made in korea as most of you remember "Lippen-10" for penile shaft girth enhancement and there is now new one for Glans enhancement it is called Lipen-P) :-

www.ec21.com/product-details/FILLER-Lipen-P--7271558.html .

The injection site is just near to the urethral opening, you just need two injections one on each side of the urethral opening :-

www.faqs.org/patents/imgfull/20130066148_01 .

They have found that the lamina propria (where the PMMA it should be) at this area forms the thickest layer:-

www.faqs.org/patents/imgfull/20130066148_03 .

This is the paper :-

I have received today an e-mail from Dr. Dae Yul Yang I asked him about Lipen-10 and Glans enhancement (esp. the new technique mentioned above) :-

Thank you for your concern about Lipen.

Lipen is nowadays very popular tissue filler in Korea.Mainly this material is using not only for penile/glans enhancement but also for facial filler in cosmetic purpose.

As for glans penis augmentation, Dr. Cho(This email address is being protected from spambots. You need JavaScript enabled to view it.) who is the developer of this material and is also an expert in regard this procedure.

I as a PI for clinical study for this marterial would like to recommend you to contact Dr. Cho. Best, Dae Yul Yang.

I have sent an e-mail to Dr. Cho already asking him about (pic.s before and after, the cost, the number of sessions, the injected volume in each round, the conc. and the complications ????

@smartman
You are definitely deserving of your name and it holds promise that there will be a valid procedure one day for glans enhancement so we're on our way... Regarding lengthening aside from the mechanical means it looks like nothing much holds promise unless you buy into the chemical dream. I'm almost over that phase.

Hi Smartman,
You will see in my post operative progress report that I was thoroughly examined by a neuro-Urologist who rubbed around my instrument taking special care to for some reason massage the glans. I wondered why but while he was so doing I realized how small it is compared to the enhanced Girth of the shaft and find that I cannot justifiably go for any more Girth if the glans doesn't get at least a temporary boost in diameter. This is why I am quite focused on this semi-permanent solution to not have a repeat procedure each year. Let me know if you hear anything new or I might just head out their myself and buy their sales pitch..

Hello friends,
these interesting novelties but the glans spongiosum is extremely delicate and vascularized.
The intention of using Hyaluronic Acid in the beginning was to treat men with primary premature ejaculation problems, because the injection of Hyaluronic Acid would decrease the sensitivity of the glans.

I am awaiting news on the application of a biodegradable polymer that can be used for this purpose but this is nothing certain yet.

Dr Witehead NY Alloderm applies to enhacment glandular but never saw one member of PE forum to show how it is after the enhacment and frankly I do not know how Alloderm could help enhacement glandular.

I hope nobody is putting the cart before the horse. I'm in agreement with the others that this is probably not something I'd be interested in. Regardless, I just spoke with them both a couple days ago and this came up. It very much sounded like they are going to learn about the technique...whether they are going to offer it would depend on what they see obviously. It certainly didn't sound like a foregone conclusion that this would ever be offered at all. It sounds just educational at this point.

On the other hand...if someone was hell bent on wanting it ASAP what stops you from going to S. Korea yourself to visit this dr who is already experienced at it?

HC,
I have just spoken to Dr. El-Seweifi on the phone (he was very nice and he answered most of my Q.s) he told me :-
I have used Artecoll injection for years but the results will stay only for one year and he prefers the DFG; he inserts it through two lateral incisions . He has stopped doing Artecoll for a year only because of its duration, but he told me if you want Artecoll I can do it for you ??? He had no cases of Necrosis or other serious complications from it.
I was so surprised with his statement I told him PMMA is a permanent material , and I have Artecoll in my body for 15 years, I told him about the Korean paper that the injection should be superficial and not deeper than 3 mm (deeper than that you will inject PMMA into the Corpus Spongiousum) .
My conclusion in regard of his Artecoll's injection is either :-
1- ?? His technique :- he is injecting the beads deeper than it should be and implanting the beads into the blood ???
2- Or less likely there are no or little new collagen formation around the beads in the laminae propria of mucosa: I doubt that .
The only good thing I conclude from this is:- There was no serious complication from injecting PMMA into the Glans, so it can be done but the only Q. is how long it will stay, is it the Technique ???
I would like if somebody can show Dr.C. (during his consultation with him) about these 2 points and to know his opinion :- 1- The Korean paper in injecting PMMA into the laminae propria of the mucosa of the glans .
2- And also about Dr. El-sweifi Artecoll injection.
I know that he will still refuse doing it , but there is no harm of showing him these informations .
Thanks

I have asked Lemperle's opinion about Dr. A.El-seweifi's Artecoll injection results :-

"You are correct ! He either used another product (Radiesse or Sculptra) or went too deep into the spongious tissue...
We all know that dermal fat grafts will be absorbed over time...otherwise, we ALL WOULD DO IT ! "-

I know some people wouldn't agree with Lemperle but I just want you to know all the information I get .
I have also sent an e-mail to Dr.Samy Passy in Rio. about the Korean paper and also about the doctor in Berlin , I just want to know his opinion too.
I would like to know what is Dr.C.'s opinion about these informations (in Korea and in Berlin) ---> no serious complication ???
Did he ever try it ? Did he inject e.g. a very small amount of Newplastic in a small area of the corona???

Yes, if I recall it was not PMMA that was used in this technique. But I thought Dr C said the product did have a small amount of PMMA in it. And Lipen does sound familiar so I believe it must be Lipen-P. But he did say that is does go away over time. Sorry, I can't be certain, at the time it was all new to me.

I have sent an e-mail to Lemperle about this doctor's technique in using Artecoll for the Glans and that was his reply :-
I know Dr. El-Seweifi well - but never met him in person, because I asked him some 10 years ago to publish his method of wrapping a non-absorbable net (mesh) around the penis in my book on Aestetische Chirurgie (aesthetic surgery). Concerning a glans augmentation, my feeling is to augment the corona only to make it fit to the shaft. This can be done by Artecoll, as it has been done with a temporary fillers before , but it must be applied very superficially - because the rest of the glans is a spongy tissues , where you may get intravascular and may inject it into the blood stream. However, Artecoll is not anymore on the market since the Chinese bought the factory EMCM in Nijmegen, Holland. Dr. El-Seweifi could buy the Artecoll directly from EMCM, as I know. Best, Gottfried

We started clinical trials of Glans enhancement using Lipen D to get approval from Korea FDA. It is go along under management of KFDA.
I think non-biodegradable PMMA materials is not suitable on glans enhancement even if there is previous successful experiences.
<Reasons>
Since the target area of injection is connected right with Corpus Spongiosum without anatomical septum , I suggest it will not necessarily be safe even though the particle size of PMMA 100 micon.*And cross linked Dextran could be long-lasted in the human body, it remains stable within the collagen tissue capsule at any cost, and if there is no tissue capsule, it biodegrades. I have many successful experiences about it.

Hi Smartman,

This isn't quite the good news I and others here had hoped for! At least there is some progress in glans enhancement research and applications. Two things irk me about Lipen-D:

- cost: roughly $4000 to $5000 for 4 tiny shots in the glans during a brief outpatient procedure. This product is outrageously priced, even if the research, patenting and clinic time is included. One milliliter is a very small quantity. He also is quoting the amount of product meant to enhance a Korean, when it is rather well known that in the ethnic distribution of penis size, they are at the lower end of the spectrum. We can assume that we might need between 10 and 15 milliliters, which could make the cost skyrocket. That is a heavy budget for a small portion of our anatomy, especially as this isn't a one time only procedure?

- permanence: it only lasts 2 years? So this is just another resorbable filler which at least works without flowering or adverse reactions? That is a long way from a permanent solution such as PMMA. I hope that one day they shall find a way to permanently enhance this part of our anatomy. There must be some material which can be used similarly without serious health or aesthetic risks. BACK TO THE DRAWING BOARD...

I had understood that they had come up with a breakthrough, but the PMMA breakthrough for shaft girth enhancement may have led to excessive expectations among us. Actually, clinical dextran isn't as expensive as one might think. Non clinical is rather cheap when compared to the prices charged, plus the clinical cannot correspond really to what is billed when you consider that it is supplied in quantities from 100 grams per unit and as high as 50 kilograms of powder in a single package (as large as a cement bag). Let's hope such high pricing at the only clinic in the world offering this procedure doesn't lead the daredevils among us to inject themselves !

www.dextran.net/dextrans-clinical-dextran.html

Dextran is a branched glucan composed of linear ?(1?6) linked glucose units and ? (1?3) link initiated branches. Dextran ranges in size from 10,000 to 150,000 Kd. Dextrans are used in many applications as volume extenders, stabilizers, matrix components, binding platforms, lubricants and physical structure components.

SOURCE: Sigma Aldrich Dextran from Leuconostoc

I really hope they also propose Lipen-P which makes the procedure and travel around the planet cost-effective if not done every 2 years... Keep us posted when you hear back, and many thanks for saving us all the footwork to find this out for ourselves!
