12 years 7 months ago #1280875059Going back to fill in the last few divots and hopefully another Kenalog shot to finally get rid of the Nodule completely.
However my skin is still stuck at the base and this really sucks as i'm uncut and when i retract the Foreskin it ends up lumping at the base. Going in for one more round and hopefully they can figure it out otherwise I will have to go to a Urologist who can try and remove the PMMA that is causing the problems. I might inquire about Dr. Camacho who has done scar Removal. Maybe he can do PMMA Removal where my skin is stuck. Does anyone know of any doctors who have done reconstruction for PMMA patients in california?

12 years 7 months ago #1280877224I think the best option to is release the adhesion with a needle and then continually manipulate the skin. Alternatively Dr Comacho might be able to access the adhesion via his scrotal incision method. This means you wont have a circ incision or an incision at the base, so you should be able to manipulate the skin form the moment you come around from the anesthetic. Once it's free you need to keep it away from the shaft and moving as much as possible. If you just let it rest, it will simply re attach itself to the skin.

I do know Dr. Camacho uses the scrotal incision to remove scar tissue from other procedures so i was thinking he might be able to get to the troublesome PMMA. I don't necessarily need PMMA Removal, i just need the skin to become detached so it can move properly.

In my opinion you can't just shove a cannula in and out in the hope of releasing it from the skin.
You need to spend a lot if time finding the exact point of the adhesion. It will seem that it's only stuck in a smaller area, but if you stretch the skin, I strongly suspect you'll find the adhesion is over a greater area than you believe. You need to keep the skin stretched taught and lifted from the shaft. Then a needle (I think an 18g Nokor is ideal) should be used to kind of scrape the adhesion of the skin. Once it released you need to do everything you can to stop it rejoining, so you'll have to try and constantly manipulate for a couple of weeks.
I bet Dr C has never actually released the skin properly. During the procedure did he actually show you the skin was free?

12 years 7 months ago #1280881369the other issues are definitely resolvable and they should be fixed by this next appointment. Still a little divot on the left and a Nodule on the upper right. I think with proper after care the issues can be avoided. My divot and Nodule is because I massaged Erect with my skin retracted. When you are uncut the Foreskin is substantially tighter then the rest of your shaft skin. So while I was massaging with my skin retracted I was inadvertently squeezing PMMA away from where my forskin fell. This created a divot in that area and pushed PMMA to the upper injection site causing the Nodule. Round 2 i massaged with out retracting and it worked out.
Still not sure how the base skin became stuck to my shaft. I think it might have to do with how loose my base skin is. I can feel PMMA in my skin that then leads back to my shaft.

But a cannula is blunt and a nokor needle is like a mini blade.
A soft tissue adhesion is a tricky fix. Finding the exact area the is stuck and then freeing it with damaging the skin or veins is hard. It's really fiddly and I think next time you go, you should get Dr C to focus purely on that. I don't think he'll be able to do it unless he locates the exact area, creates a tight skin "tent", for which he'd probably need Wade and some surgical tape. He's then got to carefully scrape or cut the soft tissue from the skin and it will require a sharp edge. Even then it might well just rejoin.
I don't know if it would work, but if he gets it free, maybe if he injected a tiny bit of Restylane under the skin, to sit between the PMMA and the skin, it would stop it rejoining.

12 years 7 months ago #1280881841I do have insurance and I'm trying to see if I can get Dr. Gelman from UCI to possibly try and unhinge the skin as I attend UCI already. If so then I'll probably just have Dr. C focus on filling in the divot and reducing the one Nodule and then see if Dr. Gelman can go in afterwards to unhinge the skin. I agree that the freshly free skin will probably reattach. I plan to at least keep it lower at the base so it attaches lower. I know sometimes in lig cuts they'll place fat in-between incisions to prevent attachment. I'll ask both Dr.'s their opinions.

No, I've just had lots of surgeries, most of them with poor results, which leads to hours of research on what I can do to fix it.
I'd imagine Gelmans solution would be degloving, release of the adhesion and rotation of some Dartos fascia or putting something in between.
