TOPIC: Imperial's PMMA Progress Report

I had my first session with Dr. C today and I couldn't be happier. I was really nervous going into it but everybody was great and made me feel comfortable with the process. Considering I was having somebody take pictures of my member (I was most nervous about this) and then having two guys man-handling my member this was no small feat. It's been 6 hours after the procedure finished I'm really excited by the results. I know that I can expect the results to shrink down and then slowly grow back over 6 weeks. But if I get close to where I am now after the first session I will be happy. I am already planning to come back in 8 weeks for round 2.

For a little background, I am 32 years old and have never posted on this site before. I have been a member for a couple of months and this site was really helpful in me choosing PMMA with Dr. C. I have looked into Phalloplasty off and on for a decade. Although I've always hated my size, I am a conservative person and the options never seemed safe or effective. The feedback and support from members on this forum were really helpful and in choosing PMMA.

I'm still new to the jargon people use to describe their measurements but I know people will be curious so here it is. Prior to today I was 5.75" Erect, or 6" bone pressed Erect. Girth was 4.25" at the bottom and 4.5" just below the glans. Flaccid state looked small and in high school and college a few guys flipped me crap in the shower. I guess that's why I was so nervous about whipping it out. The good news is that I already feel "heavier" and look much better in the Flaccid state. My Erect Girth is also noticeably bigger as well. If people are curious, I'll post the final stats around the 6 week mark. I did 16 CC at 20% and 1 CC at 10% near the top. Depending on Aesthetics I'm hoping for an 5.5" - 5.75" in Girth at the base and 5" - 5.25" below the glans.

Hi Imperial. I too am new, researching, and watching & thinking. My stats are almost identical. Would/have you thought about the lig surgery? As I am thinking of it followed in a few months with the first round of PMMA.

I have looked at the lig cut surgery before and decided it's not for me. I certainly wouldn't mind some extra length but from everything I've read the lig cut doesn't increase the Erect length, just the Flaccid length. I'm on day three and despite my best efforts there is some bunching of the Girth around the middle shaft. I think this is due to the penis retracting and yes it looks a little unnatural right now. I'm not too worried because I'm going in for a follow up in 8 weeks which should help quite a bit. I think the lig cut might prevent this and maybe give a more aesthetic look.

As a happily married guy I care more about the "end result" and not as much about how it looks. If I was single I would probably be more concerned with the look and might consider the lig cut procedure.
For me, the risks to EQ and other side effects totally outweigh the benefits.

I am looking into a PE regimen before going into my next round with Dr. C. Probably ease into a light stretching and Jelqing routine. I've always been skeptical this could work and was worried about hurting myself. But from what I've read on this site and others it really can work. So I figure I'll give it a try.

Yes. All info. helps. I want realistic and aestheticly natural looking and WORKING results. I would never modify factory 13" wheels to 30" spinners... maybe 20's or 22's.

Well it's been almost a week now so I figured I'd post an update. I'm definitely noticing some "shrinkage" due to fluid re-absorption. I knew it was coming but it still sucks.

The pain and swelling has mostly subsided except for around the fat pad. The soreness is finally starting to subside and it remains a bit swollen. I'm not sure if it's the swelling or something else going on because I lost 1/4" of length since the procedure. I'm going to start stretching once all the swelling is finally gone to try to get it back.

I also have a bit of swelling of the Foreskin around the glans. This has gone down quite a bit since the procedure but is still there. I asked Wade if this is part of the procedure or if it will go away.

Despite all that I'm still really happy I went with the procedure. I've already scheduled my next visit for 7 weeks out!

Tomorrow will be four weeks and other than a few aesthetic issues, things are going great. My Erect length is almost back to normal which is a relief. My theory on this problem is that there is swelling in the fat pad from the procedure and that once the swelling goes down the length comes back. I'm wondering what others think of this.

I don't know how accurate my initial measurements were but my current stats from last night are 5.125" BEG, 5" MEG, and 4.5" just under the glans. This has to be at least 1/2 an Inch from starting stats but feels like more for some reason.

I tried to post some pics but get the Storage limit exceeded problem.

On a side note, I tried on a Magnum last night and it actually felt pretty snug. Does anybody know what the "range" is for a Magnum? Is 5" EG enough to safely wear one?

Okay I jumped the gun... Lol. I checked the size chart and 5.5" is the minimum for magnums. I know I read that somewhere else on the forum... damn wishfull thinking. I'm assuming the snugness is due to the fact I have always been indersized and am not used to a standard condom being so tight.

I checked the Kinsey report and I went from low end of average to average in round 1. Hopefully round two will get me to the high end of average.

I thought it was closer to 5.25" ? If it's snug and placed on correctly, I can't imagine it being more dangerous to wear than a regular condom with a tight fit.

I think we are all in the same boat re: starting stats. I'm just shy of 6" EL, and about 4.75" EG (a bit thinner at the base). I've also been thinking of the lig cut, then PMMA. Aesthetics are key to me, and I'm not actually looking to get a big Dick - just something more comfortably in the "normal" range (rather than on the edge of it...).

Baywoof - have you seriously looked into the lig cut? I think Gary Alter sounds like the most reputable - any thoughts on doctors? I'd be happy with just Flaccid length gains, although I think the difficulty in getting some Erect gains is largely a function of people not being rigorous enough with the post-op stretching regime.

I think you got me beat in starting stats. I was initially measuring with a string and not a tape and that's how I got to 4.5" MEG. I think I was probably closer to 4.25" (call it 4 1/3" ) MEG now that I have measured with a tape post-PMMA. That was why I made the comment earlier about being on the low-end of average. As I stated earlier standard condoms felt loose on me so I must have been less than I initially thought. I can't say anything negative about my PMMA experience and am extremely happy I went through with it. But than again, I was suffering from SDSI (Severe Dick Size Insecurity ) and was willing to accept the risks of a relatively new procedure. The aesthetics are not perfect. I have nodules and ridges along the circ scar that need to be taken care of. It also does not feel like my normal penis yet. Flaccid it does feel a little rubbery and there are imperfections as well. When it's erect it feels pretty normal to me only harder (which I think is better).

The only advice I can give is to read EVERYTHING you can on PMMA from this site before making your decision. Then if you do decide to go ahead, make sure to go to Dr. C. I wouldn't trust my junk to anybody else.

Round 2 was yesterday. I flew into San Diego and and took the shuttle to the office. The appointment was a little over two hours and then I went straight to the airport and flew out. I used my my frequent flyer miles so the total cost of the trip was $2140. It was$2000 for round 2, $100 for the shuttle, and $40 for food and celebratory beers. Wade was pretty happy about the results from round 1 and asked what my post op was. I told him I just followed his instructions and constantly massaged my penis for the first 48 hours. I can tell he is really trying to figure out the best post-op routine to follow. We talked about the silicone sleeves and he said they have seen really good results and really bad results. I'd heard someone post that before on the forum so I asked him what went wrong. He said that some guys just didn't pay attention to it. The sleeve would roll up and cause really ugly problems like a big crease on the shaft. He also seemed interested in the jes extender. I'm thinking this may work as the pictures on their website show a plastic ring just under the glans and not on the shaft. Anyone have success with the silicone sleeve? I think I saw a picture of JMLB with one on, maybe I'll ask him. Anyway… prior to the procedure we talked about some of my issues. The worst of theses were that I had quite a bit of nodules and ridges in my circ scar. They weren't visible with an Erection or painful during sex but when I was masturbating and rubbed them the wrong way they would cause a pain in my glans. Dr. Casavantes seems to follow the less is more philosophy. He mentioned that when he had his second procedure (performed by Wade) there were some decent size nodules on the top of his shaft. He said after 8 - 10 months they were almost completely gone and no longer visible. He mentioned that he avoids kenelog injections whenever possible because of potential side affects like skin Atrophy. During the procedure he went in and tried to break up the ridges with the microcannula which seems to have made a difference. I can't tell for sure because there is a bit of swelling in my circ scar. The second problem was that I had a bit of a bulge on my right side. This was only an aesthetic issue so I wasn't bothered by it. We'll see if they were able to even things out better in round 2.The third issue was the gap between the shaft and circ scar. They had problems getting product to stick in this region in round 1 and warned me I would have a gap in this region. They had much more success this time around getting the product to stay. Wade gave me explicit post op instructions on how to push the PMMA from the shaft towards the circ scar. The problem is that when I retract the PMMA gets pushed right out again. Currently I have the shaft wrapped and just continue to push the PMMA back up the shaft every 10 minutes. A couple of things to note: There is quite a bit more bruising this time around. They warned me this would be the case yesterday and I think it might be related to trying to break up module on the circ scar and the bulge on my right side. They made four incisions this time. Two at the base and two mid-shaft to fill in the gap. There was more pain this time around. The first time I felt nothing, this time they had to continuously keep injecting lidocaine. I know Smartmen and one other person had this issue has well. I mentioned this to the doc and he thought that was interesting. He said most people had more pain on round 1. As I said above I went straight home after the procedure and had Wade's wrap on it for about 8 hours. It worked great and there were no irregularities when I got home. One thing that may have helped was that I wasn't retracting because I took a percoset at the office before I left. I got the idea to use percoset when I was taking it for back pain. When I took one for my back pain it helped with retraction. So yesterday after the procedure I asked if it would be okay to take one (should have taken on prior to the procedure as…lol). They said that it would be fine and would not interfere with anything they had given me (the relax pill, the cortisone injection, or the Cipro). Dr. C explained that opiates do a good job of preventing retraction but have the side affect of interfering with erections. This worked well for me as my wife wouldn't let me order alpha blockers because I have hit my yearly Cock-spending limit…LOL. Well sorry for the long post but I wanted to get this all down before I forgot. I have to say that I am just super impressed by the staff at AvantiDerma. The atmosphere is great and everyone is super chill nice. You tell they really care about their patients are constantly perfecting their techniques. Next steps are to take a 12 month break from PMMA and trying to get some length. I'm still trying to figure out if I will start Hanging or using the Jes Extender. Probably wait 2 or 3 months to let things settle and get after it.

Day 8 post-op round 2

My earlier post about no aesthetic issues was pretty far off base. By days two and three my penis swelled up and turned into a frankendick. By day 5 I was getting pretty worried but thankfully Wade called to follow up. I sent some pics to him that night and he called back the next day to discuss. I had three issues that were worrying me. The first and ugliest problem was that the underside of my shaft near the glans was way bigger then the top side. This made my Dick curve upwards at the tip and just didn't look right. I thought this was PMMA and would be there to stay which is why I was so worried. Wade told me they have seen this before and that it was almost assuredly just fluid. It will take a while to get reabsorbed but is temporary. I'll make sure to follow up on this post to let everyone know.

The second problem was caused by the first problem. All that fluid stretched my skin so tight that it caused a crease on the the top left side. I don't think there is anything to do for that but have a touch up round. The third problem was that there seems to be a more PMMA at the top on my mid shaft. This gives the penis a bit of balled up look and Flaccid and an hour glass when Erect. Wade said that a lot of that could still be swelling because I did bruise pretty easy there. While it would look better if things were more even it would suck to lose the size there.

I think I will definitely need another session to get the look I want. Right now I'm at 5.5" mid-shaft and I think that is my perfect size. I would like to wait 6 months to get to my "final size" before going in for round three. But it depends on how things end up looking in 4 weeks. If things settle in nicely as Wade is hoping for I will hold off. But if the gap is really pronounced I will go in sooner.

The other reason I would like to hold off is to see how things are with my wife. Currently she is getting sore every time. Even using lube she is still sore for a day or two after. She still enjoys the sex but we are trying to get used to my new unit together. It really bugs my that I'm causing her pain. 5.5" is definitely my perfect size but she may actually want things to shrink up at the 6 month mark.

Two thing I would like to point out is that dr. C got rid of of the ridges in my circ on the right side (left side is filled with fluid so I can't tell). He just went in and broke it up with the cannula. I think it's great that this technique is so effective. The second is the fact that Wade called to follow up on his own. I thought this was really cool of him to do that I felt much better after talking with him.

I'm still on the fence about posting pictures. If people thought it would them I probably would but I don't like the idea of having my Dick posted on the web...

Almost 3 weeks post-op round 3 and things have settled down a bit. The aesthetic issues are not as noticeable now but I still wouldn't feel comfortable taking a shower in public. I haven't measured in two weeks. I know things shrunk down and should be hopefully starting to build back up now.

I am looking for advice from others about when to have round 3 done. From what I've read from others, it would best to let things settle for 6 months to know what the final state will be from round 2. I would prefer to do that but the remaining issues are kind of annoying to me and I think they would be resolved in a third round.

I have some PMMA built up in the left side of my circ scar. Last time the "ring" was on the right side and Dr. C was able to break up 95% of it so that it is hardly noticeable now. That and the gap on the top left of my shaft make it apparent that something is not quite right. I also need a bit more PMMA on the middle underside of the shaft and all around the base.

Does anybody have an opinion on whether waiting would make the next round more effective or if I should have it done sooner?
