It's not filler, like alloderm or PMMA, it's your own pubic flap, so, it can not reject and girth gain can be significant, for example on 1.5-2 inches. The trouble with the flap technique is that it is still a graft on a dynamic organ and these don't work well. Also, other than increased blood supply, it's not different to a dermal graft, so there is still all the risks of surgery, plus a big scar.
