I think it comes down to testing and legality. In countries where it is legal girls are more professional and treat it as a career to an extent. Therefore they get tested for STD's regularly. I doubt the average girl in a club gets tested more than once a year. And as you say, it's simply in the escorts best interest to be as hygenic and safe as possible. The only "stretcher" I have used would be the vacADS from autoextender.com, and its not even really a stretcher as it is a vac head-piece and a cord.
