I understand the point that it's microscopic, but it looks like once formed, the collagen creates a similar single mass of tissue much like the free fat. Kind of a large plastic filled tumor. Maybe it is much harder to remove individual portions through punch incision, like you said because that tissue is not natural and compromises the healing, but if they went in and separated the entire implant from the natural tissue, things would heal relatively normally. Also, in that picture I don't understand why they made the incision on the top of the penis. Who knows the particulars and maybe it had to be done that way, but an underside incision along the seam would be much less disfiguring, and hopefully it could be done like the fat removal and just deglove along the circ scar. Here was what I had written before, below. I just learned that stuff from my doctor in the last month or so. Also, on the show on E! called Botched, the surgeons (Dr. Dubrow- who does the boob jobs) always talks about how there are cases when the woman goes too big - you can see the blood vessels are really unhealthy and dilated all over the breast. They only blur the nipple out so you can see it in the show. So, I asked my doc if he thinks that's why PMMA caused all these weird little capillaries to form and he just said it's a good theory (I'm like 1 of 2 PMMA cases he has). The theory is just that all the surface blood vessels in the skin and Dartos fascia aren't functioning well due to the PMMA, so the body sends signals to make new ones. -- old post-- PMMA gets stuck in the tissue (more the dartos than bucks) and also stuck in the wound, so the wound cannot close. The dartos is responsible for lymphatic drainage of the penis, which is why so many guys with PMMA have extended periods of swelling after sex. Their lymph channels are damaged. Who knows what the long term meaning of that will be, but it's generally not a good idea to clog your lymphatic channels. Additionally, PMMA causes capillary dilation throughout the shaft, which you also see in breast augmentation patients who went too big. The skin is stressed and not getting adequate blood supply. It doesn't just come out out. It has to be dissected out - and even the most senior surgeons at the most famous hospitals find this difficult (worse if it has been in longer, it seems). PMMA is microscopic and fat is macroscopic... so fat is far easier to remove. My surgeon was frustrated by how this stuff spread under the skin and would cause wound healing problems, though I think he's now one of the few experienced doctors out there with PMMA. Regarding surgeries like this: If you had rock hard jagged edged lumps in your Dick and were single, you would probably make up your mind to get it out. It actually hurts to have these nodules. It's not natural or normal in any way to have sharp edges poking through the skin of your Dick when you are Flaccid - and it's 10x worse Erect - you know during sexual activity when there is friction (against jagged edged lumps under your penile skin). It is uncomfortable at a minimum and painful at a maximum. And, that only gets worse with time. They're not round. They have sharp edges (but even if they were round it's not pleasant.) If you only have 1 or 2 problems, you don't have to do something this aggressive. It would've been a much better idea if I had that surgery in that video rather than going from 1 Nodule Removal to the next half of which had to be redone because the skin/dartos were too compromised by the PMMA to heal). But, since I was the first one to openly post about having nodules removed without a deglove, nobody could ever have predicted it would've caused wound healing issues to the extent it did - and end up being a total shitshow of multiple surgeries. PMMA patients who had facial nodules removed did not have these problems either. The penis obviously is very different from the face in many ways, as is the placement of PMMA. Ian Eardley once told me people who are Circumcised are more likely to lose length by having a Removal op and from this video I can see why, imagine if you get even a fraction of tissue Necrosis it would be so much harder to deal with with such a small amount of skin. The complications Resto had from these correction surgeries are skin loss and length loss. I bet these complications would have been less severe and more manageable had he been uncircumcised.
