You can do this while also stretching the penis a bit by moving your hands away from the base a bit while rolling and applying pressure. Obviously, if you have a lump in one area, you can use your thumb to spread it out, but in general the "palm rolling" technique really distributes the filler evenly through the shaft. There were two methods. One was to roll the thumb over the penis from base to head in progressive movements. the second was to use the supplied plastic roller and do the same. The roller was maybe 1.5" long and 1/8" in diameter.
