(I got filler this summer, and it's pretty lumpy. Supposed to go back for round 2 later this year. Curious about how this has been dealt with for other people. Wondering how different doctors have handled it.) Hard to give relevant comments without knowing your situation. Mine was not quite lumpy but def settled in just a few areas. On my second procedure the doctor added in filler in the area where there was less filler. I have a clump near the base of my shaft but it’s not visible as it’s all formed into the proper shape, but I can feel the filler clumped up in that spot. I massaged the hell out of my filler and used “athletic under wrap tape” (Google it) to keep it in shape or keep excessive inflammation at bay for several months after. So even the area that has formed a clump is smooth and shaped properly. Without the additional round of filler, my resound would still be uneven. I've never heard of using the cannula to "break things up". As stated by other posters, wrapping and massaging (to a limited degree) is important. Those who get fillers should be massaging ( aka - rolling/forming & maintenancing ) their penis FREQUENTLY and with heat as well. i wont get into the how to's here as I have written about this before. But it is imperative to achieve a good result. what should be limited is the compression - IE: wraps and tapes and anything that compress the shaft at all.
